From my perspective as a surgeon, when I do a laryngeal tieback, I use
antibiotics post-op (prophylactically) if my suture penetrates the lumen of
the larynx and no antibiotics if it does not. Logic being that the larynx
is not sterile and if the needle passes thru the mucosal lining and then
back into the tissue of the neck, it can bring bacterial contamination and
start a local abscess. My goal is to not have it penetrate the lumen, but
it is not a "big deal" if it does.
Obviously, if pneumonitis/pneumonia is present at the time of surgery,
appropriate antibiotic use (therapeutic) is advised.
